If you know a woman who, in your estimation, has made history through her efforts, the Women's Center at Oregon State University wants to hear about her.

Each year, in conjunction with National Women's History Month, the women's center at OSU presents the "Women of Achievement Awards" to honor the contributions and commitment of outstanding women on the campus and throughout the state of Oregon whose work has significantly benefited women.

Completed forms must be sent by Friday, March 5, in care of Beth Rietveld to the OSU Women's Center, Women's Center, Benton Annex, Corvallis, OR 97331-2503.

The committee will be making final decisions by March 12, and notifying successful nominees that week. The awards will be presented in a ceremony from 3:30 to 5:30 p.m. April 23 in the Memorial Union Lounge.

Examples of criteria considered in presenting the award include a demonstrated commitment to equality and dedication to furthering the status of women in Oregon; engaging in work that has positively affected the lives of women and has potential for creating real change; serving as an important role model in the lives of girls or women; making progress on issues such as pay equity, violence against women, women's health, etc.

Nominators must provide their own name, address, phone and e-mail; state their relationship to the nominee; the name, address, and e-mail of the woman being nominated; and provide a letter that details the specific examples of the nominee whose actions make her a fit for the criteria listed above.

Nominators may also include up to three additional letters of recommendation from other individuals who also would provide nomination criteria of which the letter writer has direct knowledge.

A curriculum vita or resume for nominee also should be included.
